Please bring your medical/Medicare insurance card, and prescription insurance card.  Please plan on arriving 5-10 minutes prior to your scheduled appointment to fill out a quick vaccine consent/questionnaire. 

Most insurances fully cover the flu shot.  If you do not have insurance regular dose flu shots are $50 and high dose formulations (65 and older only) are $105.

Our pharmacists will attempt to perform a vaccine assessment to identify any other potential vaccine needs for you!
